FORT LEONARD WOOD, Mo. (Feb. 2, 2021) — General Leonard Wood Army Community Hospital is hosting a COVID-19 mass vaccination event from 7 a.m. to 7 p.m. this Friday (2/5/21) and Saturday (2/6/21) at Nutter Field House as part of the installation’s second phase of the vaccination rollout plan. The goal of the event is to provide a wide distribution of the first dose of Pfizer vaccines to Fort Leonard Wood service members, Department of Defense civilians and contractors, and enrolled beneficiaries who are 16 years of age or older.
Appointments are required and can be made by visiting https://informatics-stage.health.mil/GLWACHCOVIDApp.
Please complete the COVID Vaccine form and bring it with you during your appointment; vaccine forms are available at: https://leonard-wood.tricare.mil/Health-Services/Preventive-Care/Immunizations/COVID-19-Vaccine-Program
IMPORTANT:
If you have been diagnosed with COVID-19 within the last 90 days prior to Feb.5 or Feb. 6, you will be considered ineligible to receive the vaccine at this event.
If you have had another vaccine within 14 days of Feb. 5 or Feb. 6, you will be considered ineligible to receive the vaccine at this event.
The vaccine is only approved for those 16 years of age and older.

1) We are very excited to announce we will be offering the COVID-19 vaccine to all TRICARE beneficiary types (to include young adult, Select, Prime, TRICARE for Life, Plus) during our Mass Vaccination this weekend!
2) If you are able to access the following link for the screening form, please bring it with you otherwise we will have copies to fill out.
Appointment confirmations are not available using the scheduling tool. If you weren’t able to screen shot your appointment, don’t fret, come to the event anyway! We are excited to provide you with the vaccine!
3) Please bring your military/beneficiary Photo ID.
